Bristow Case Management can help providers more effectively utilize their internal case managers by identifying high risk patients and assisting them with gaining access to the care they need including making face to face home visits 7 days a week.
In regards to Quality of Care, we can help providers:
- Increase access to pre-natal care
- Reduce the scope and severity of post-partum depression
- Improve HEDIS measures
- Meet or exceed NCQA standards
- Reduce emergency room visits and hospitalization
If you qualify for Medicaid, we offer a large variety of case management services to you at no cost including:
- Coordinating services if you have a child or children with disabilities
- Connecting you with helpful community services developed specifically to help pregnant women or children with special needs
- Personal visits to your home by our licensed nurses or licensed social workers
- Assisting you with getting emergency assistance including rent, utilities, food, and other basic needs
- Helping to determine if you are eligible for special home care services
- Helping to coordinate service provision with physician healthcare providers including primary care, OBGYN, pediatricians, neurologists, pulmonologist and psychiatrist
